Together Home
Plus Community delivers the Together Home program across Bathurst, Orange, Dubbo and surrounds, working with people who have experienced long term homelessness or rough sleeping.
The Together Home program aims to transition people onto a trajectory away from homelessness and into long-term stable housing, while improving participants overall personal wellbeing.
Together Home Can Help People Who Are:
Being supported in Temporary Accommodation.
Interested in moving into long-term housing, but may require support to do so.
Over 18 years old.
Clients of Specialist Homelessness Service providers who are currently in crisis or transitional accommodation.
Australian citizens or permanent residents and eligible for housing assistance.
Street sleeping (or living in tents, cars) or have a history of street sleeping.
Known to Assertive Outreach services across NSW.
How Does Together Home Work?
Entry into the Together Home program is by referral. The program will help you find stable accommodation. The support coordinators may also be able to support you with:
- Health issues including physical and mental health
- Financial issues, debts and legal issues
- Living skills, like cooking
- Connections to community and family
- Connections to culture
- Positive activities and hobbies
- Education and employment
- Your personal wellbeing
